All care decisions must come from a shared partnership between the professional and the child, young person or adult. But for those who do not have capacity for their choices, or may lose that capacity in the future it is important that the the right choices are made. Deciding right is a north east wide initiative - the first in the UK - to integrate the principles of making advance care decisions for all ages. It brings together advance care planning, the Mental Capacity Act, cardiopulmonary resuscitation decisions and emergency healthcare plans. Written by health and social care professionals, Deciding right identifies the triggers for making care decisions in advance, complying with both current national legislation and the latest national guidelines. At its core is the principle of shared decision making to ensure that care decisions are centred on the individual and minimise the likelihood of unnecessary or unwanted treatment.
